Those things suck and often bug out if you have an adblocker, or they don't accept attachments, or never send you an email confirmation, or have AJAX errors, or block part of the screen, or only give you 3 prefilled options that don't match the actual thing you encounter...
They are easy to drop in and have better reporting than email, but IMO their UX is just atrocious. Qualtrics is especially bad. If I see a Qualtrics feedback form I just immediately close it.
