Even assuming people can see every potential issue at the design stage (because design mistakes never slip through and everyone has perfect foresight, right?), what also sucks is when you're not in the set of people asked for input in the design stage to begin with (maybe you weren't high level enough, or maybe you weren't on the project or team at that point, or whatever) and then you end up having to implement something where the design seems wrong or nonsensical to you.
