Afaiu, the solid truth was that higher-up admin people of the Parliamentary Labour Party HQ, which is separate from the office of the Leader Of The Opposition, failed to do the investigating, partly due to the fear and hate of Corbyn in the PLP, but then LOTO took the blame (partly cos Corbyn isn't the greatest of communicators).
