I've actually just given up on notebooks. They just don't work for my writing style. What I do now is use loose leaf papers and a binder. It's not as nice looking or as compact, but it lets me focus on writing instead of annoyances.

In some scientific environments (labs which haven't switched to electronic notebooks, in particular), sewn notebooks are required.
