Twilio's login form commits this sin, but goes further and tosses in a baffling "Enable 2FA" checkbox for good measure.



This checkbox is extra confusing because it enables 2FA for all users. I'm not sure how everyone else gets credentials when you do that, so I unchecked it after I figured out what I had done. Luckily, I kept the credentials in my 2FA app because someone else later re-checked it.
